ABHA (Ayushman Bharat Health Account) Address is a unique identifier ( self-declared username) that enables you to share and access your health records digitally. Your ABHA address may look like ‘yourname@consent manager’. For instance, xyz@abdm is a ABHA address with ABDM Consent Manager that will facilitate health data exchange for you with appropriate consent on the ABDM network
Linking ABHA number with ABHA address
You can use your ABHA number to seamlessly sign up for an ABHA address, and ensure that the health records created for you are shared only with you. To enable health data sharing, it is recommended that you create ABDM ABHA address and link it with your ABHA number.
Voluntary Opt-out
You can participate at your own free will and choose to create your ABHA number voluntarily. Also, at any time, you can also request for permanent deletion or temporary deactivation of your ABHA number

Terms of Use
I hereby declare that I am voluntarily sharing my Aadhaar Details and demographic information for the sole purpose of creation of ABHA ID by National Health Authority. I am aware that ABHA number can be used to fetch my health details by entities working under National Digital Health Ecosystem (NDHE) by facilities registered or linked to ABDM (Ayushman Bharat Digital Mission) as and when required by me for availing health care services.
Further, I am aware that my personal identifiable information includes Name, Address, Age, Date of Birth, Gender and Photograph which can be procured with ABHA ID. I authorize NHA to use my Aadhaar number for performing Aadhaar based authentication with UIDAI as per the Act, 2016. I understand that UIDAI will share my e-KYC with NHA upon successful authentication. I am also aware that there are other options for registration, however I consciously choose Aadhaar number for creation of ABHA ID. I reserve the right to revoke the given consent at any point of time under the provisions of Aadhaar Act and Regulations.
